Ozempic and other GLP-1 receptor agonists like Wegovy® and Mounjaro® have become popular for managing blood sugar levels and aiding weight loss.

However, some users are reporting a surprising side effect: bad breath, or “”Ozempic breath”.” While it’s not an officially listed side effect, it’s important to acknowledge that it can happen to some individuals.

Possible Causes of “Ozempic Breath”

Three culprits have been identified that could be the source of bad breath: GI issues, dry mouth, and ketosis.

GI Issues

Gastrointestinal issues, commonly linked with GLP-1 receptor agonists, may play a role in oral side effects like bad breath. While there is no peer-reviewed data directly linking these medications to oral health issues, there is ongoing research.

One notable side effect, “eructation” (or burping), has been reported by nearly 9% of people taking semaglutide. It is believed that food particles ferment in the stomach due to delayed gastric emptying, leading to unpleasant burping-related odors.

Dry Mouth

Bad breath can be related to other side effects caused by the medication. Some patients may experience vomiting and diarrhea, which can lead to dehydration and dry mouth.  A dry mouth is a breeding ground for bacteria and can cause an unpleasant odor.

Symptoms of dry mouth include:

  • Thick, stringy saliva
  • Difficulty swallowing, talking or chewing
  • A dry feeling in the throat or mouth
  • Dry or grooved tongue

If you have symptoms of dry mouth, you should see your dental professional. Dry mouth can cause tooth decay, gum disease and other problems, so it’s important to address it as soon as possible.

Ketosis

Ozempic works by stimulating the production of insulin and reducing the production of glucagon, a hormone that regulates glucose levels. Essentially, Ozempic causes your body to burn fat for energy instead of glucose.

This process can lead to ketosis, a metabolic state where the body burns fat for fuel. While some people may intentionally enter ketosis for weight loss purposes (e.g. following a low-carb diet), it can also happen unintentionally due to factors like medication use.

One common side effect of ketosis is bad breath, often described as a fruity or acetone-like smell. This is because ketones, the byproduct of fat burning, are released in your breath.

How to Manage “Ozempic Breath”

Thankfully, most of these symptoms will improve once your body adjusts to the medication. In the meantime, here are some tips to help manage and prevent it:

  • Practice good oral hygiene: Brush your teeth twice a day, floss daily, and use a tongue scraper to remove bacteria and food particles from your mouth.
  • Stay hydrated: Drink plenty of water throughout the day to combat dry mouth and keep your mouth moist.
  • Chew sugar-free gum that contains xylitol: Xylitol eliminates bad bacteria in the mouth and stimulates saliva production.

  • Be mindful of when you eat: Try to have your last meal three to four hours before bed. This gives your body time to digest and prevents food particles from sitting in the stomach overnight.

Combat “Ozempic Breath” and Maintain a Healthy Mouth

This is undoubtedly a rare unpleasant side effect, but it is typically manageable with good oral hygiene, proper hydration, and adjustments to your diet.

As your body adjusts to the medication, these symptoms may subside. However, if you find that bad breath or other side effects persist, it’s important to consult with a healthcare professional for personalized advice.
